Medical Emergencies

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Northwest Pulmonary and Sleep Medicine and they will contact us. If the office is closed, a voice message will prompt you as to how to contact the on-call physician for further action.

After Hours Urgent Medical Needs

We realize that our patients become ill at all times of the day and night. For non-urgent concerns after the office closes, simply leave a message on the office voicemail and one of our providers will call you back the following day Monday – Friday. For urgent concerns and emergencies, call the office and you will be prompted on how to page the on-call physician.

If it is not an emergency, please call our office during normal business hours. A good rule of thumb to determine if your concern warrants a call to the physician after hours is if you are contemplating seeking care in the emergency room. Requests for antibiotics, prescription refills, issues with CPAP devices, and test results are not appropriate after-hour calls.

Insurance / Payment / Billing

At Northwest Pulmonary and Sleep Medicine, we accept Medicare and most major insurance plans. Please contact your insurance company or you may call our office at (815) 477-7350 to find out if we accept your insurance plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, and MasterCard.

If you have a question related to your bill or insurance, please call our office and ask to speak to the billing department.


If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. 

Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Hospital Affiliations

Cancellation Policy

Patients who do not show up to their scheduled clinic appointment and have not called within 24 hours are subject to a fee of $50.

Patients who do not show up to their scheduled sleep study and have not called within 24 hours are subject to a fee of $100.

Prescription Refill Policy

Prescription refill requests called into the office will be addressed within 24 to 48 hours from Monday – Friday 8:30 am to 3 pm. Any requests made between Friday after 3 pm and Monday before 8:30 am will be addressed by the end of the business day on Monday.

Routine prescription refill requests will only be granted to patients who have been seen by one of our providers within the last 12 months.

Patients requesting refills or new medications for a new or acute problem will not be handled over the phone if they have not been seen within the last 6 months.

Telephone Care Policies

Northwest Pulmonary and Sleep Medicine is committed to providing you access to quality medical care every hour of the day. Telephone care is just one way that we meet this commitment.

Telephone care is the delivery of meaningful medical care over the telephone and often may replace and prevent an office visit. It includes physician management or consultation related to immediate medical needs, adjustment of medications, and treatment of chronic illness.

Telephone care can be delivered during or after regular office hours by any one of our physicians or nurse practitioners. It involves time, expertise, and documentation of the medical care delivered and is therefore recognized as patient care.

Northwest Pulmonary and Sleep Medicine reserves the right to bill your health plan for specific types of phone calls between you and a physician or nurse practitioner. You may be responsible for all or part of this fee depending on your individual health plan coverage. The specific circumstances for which you may be billed for telephone care are as follows:

Calls for physician/nurse practitioner management of a new problem, including counseling, medical management, and coordination of care not resulting in an office visit within 24 hours

Calls for physician/nurse practitioner management about an existing problem for which the patient was not seen in a face‐to‐face encounter in the previous 7 days and will not result in a visit within 24 hours

Of course, you are welcome to call for an office appointment to be seen in person at any time during our business hours. You always have the option to make an office appointment rather than receiving telephone care.
